Valid Approval Submission Checklist
All documentation that is required for any study to be considered a valid submission can be found on this checklist.

Sponsorship
If you would like NUH to act as sponsor of your research project please complete a Request for NUH to be Research Sponsor form and send to the R&D department along with the IRAS form, protocol and the other study documents.

Protocol Templates
For NUH sponsored studies you are required to use the following templates:

For Clinical Trials of Investigational Medicinal products please the CTIMP protocol template.

For all other studies, please use the Non-CTIMP protocol template.

Patient Information Sheets and Consent Forms
Please consult the NRES Guidance on Information Sheets and Consent Forms before designing your study documentation.

CV Templates
Please use the NRES CV Guidance to compose and submit your research CV. Ethics and R&D will no longer accept CVs in a different format.

NUH Peer Review Document
For studies which have not been externally peer reviewed, please arrange for the external referees to fill in a NUH Peer Review Form and send to the .
